Privacy policy.

Effective September 2026

Game Changer Physiotherapy respects your privacy. This policy explains what personal and health information we collect, how we use and protect it, and the choices you have. It applies to our practice at 6/7 Hector Street West, Osborne Park WA 6017 and to this website.

Because we provide health services, we handle health information, which is treated as sensitive information under the Privacy Act 1988 (Cth) and the Australian Privacy Principles. We take that responsibility seriously.

1What we collect

Depending on how you interact with us, we may collect:

  • Contact and identity details, such as your name, date of birth, phone number, email and address.
  • Health information relevant to your care, including your reason for visiting, history, assessments, treatment and progress notes.
  • Booking and payment details, and where relevant your Medicare, DVA, private health, workers compensation or motor vehicle claim information.
  • Information you give us through our website, forms, email, phone or in person, including marketing sign-ups.
  • Website and analytics information, such as pages visited and device details, collected through cookies and similar tools.

2How we collect it

We usually collect information directly from you, when you book, attend an appointment, complete a form, contact us, or use this website. Sometimes we receive it from others with your consent, such as a referring GP, specialist, insurer or employer for a compensable claim.

3Why we use it

  • To provide physiotherapy, massage and rehabilitation care and manage your treatment.
  • To manage bookings, payments, rebates and claims.
  • To communicate with you about your care, appointments and results.
  • To send you marketing or updates, only where you have opted in, and you can unsubscribe at any time.
  • To improve our services and this website, and to meet our legal and professional obligations.

4Who we share it with

We only share your information where needed, and we take reasonable steps to keep it secure. This may include:

  • Our practitioners and staff involved in your care.
  • Our booking and practice management provider (Cliniko) and payment providers such as HICAPS and Medicare.
  • Insurers, employers or claim managers, where relevant to a workers compensation, motor vehicle or DVA claim, with your consent.
  • Other health providers, such as your GP or specialist, with your consent.
  • Our email and website service providers, including our email platform and Google Analytics.
  • Anyone else where you have consented, or where we are required or permitted by law.

Some of these providers may store information on secure servers located outside Australia. We take reasonable steps to ensure your information is handled appropriately.

5Marketing

We will only send you marketing or newsletters if you have opted in. Every marketing message includes a way to unsubscribe, and you can opt out at any time by contacting us or using the unsubscribe link.

6Cookies and analytics

This website uses cookies and Google Analytics to understand how visitors use the site so we can improve it. This information is general and does not identify you personally. You can control or disable cookies through your browser settings.

7Storage and security

We store your information securely and take reasonable steps to protect it from misuse, loss and unauthorised access. Health records are kept for the periods required by law, generally at least seven years from your last visit, or in the case of a minor, until they turn 25.

8Accessing and correcting your information

You can ask to see the personal information we hold about you, and ask us to correct it if it is wrong. Contact us using the details below and we will respond within a reasonable time. In some cases a small fee may apply to cover the cost of providing records.

9Complaints

If you have a concern about how we have handled your information, please contact us first so we can try to resolve it. If you are not satisfied, you can contact the Office of the Australian Information Commissioner at oaic.gov.au.

10Changes to this policy

We may update this policy from time to time. The latest version will always be available on this page.
